Estrogen Hormone Replacement

THE FLEXIBLE OPTION!

         Women That realize the importance of hormone replacement therapy (HRT), but object to or cannot tolerate commercially available products may want to consider a custom compounded product.  We can custom compound hormones in either a capsule or a cream base, depending on your preference to meet your physician’s specifications. 

         We compound using plant-derived hormones which are all natural, not synthetic drugs created in a laboratory.  When describing replacement hormones, the word “natural” is used to refer to the chemical structure of the hormone, not its source.  When analyzed biochemically, the molecules of estrogens, progesterone, and other hormones produced from wild yam precursor molecules are found to be absolutely indistinguishable from those the body produces itself.  Thus the crucial variable defining “natural” is not the origin of the hormone, but its chemical structure.

          Our hormone replacement products can be compounded as a capsule, cream, troche, or vaginal suppositories depending on your specific need and/or preference with capsules and creams being the most common.   An obvious advantage to a capsule over other formulations is the ease of administration.  The major advantage to a cream over a capsule is a possible increase in blood levels.  When taken orally, estrogen is susceptible to what is known as a “first-pass” effect in which the liver metabolizes much of the drug before it is able to be absorbed into the bloodstream where it exerts its beneficial effects.  Topical creams avoid this effect because they are emulsified in a base that allows for superior penetration and absorption directly in the blood stream without undergoing immediate hepatic metabolism.

         Hormone replacement therapy (HRT) is an important component in any preventative health program for the menopausal years.  HRT produces great short-term benefits by relieving the following symptoms associated with decreasing hormone levels.  These include:

painful or uncomfortable sexual intercourse

Much has changed in the field of women’s health the past few years when it comes to the use of estrogen and progesterone for menopause therapy.  The American College of Obstetricians and Gynecologists (ACOG) recently published the most comprehensive, evidence-based clinical recommendations to date regarding the use of HRT.

HRT continues to be the most effective treatment for menopausal symptoms and should always be prescribed at the lowest effective dose for the shortest period of time to relieve vasomotor symptoms and only should be used as long as risks don't outweigh benefits. (using a compounded product makes it easy to adjust and taper the dose to achieve this goal)

HRT should not be prescribed solely for disease prevention (cardiovascular disease, osteoporosis, or depression, etc.). However, 

instances exist in which disease prevention is appropriate as a secondary benefit for women who are already taking hormones for vasomotor symptoms.

Herbal alternatives to hormones such as soy, black cohosh, and wild yam have not been shown to be effective in relieving menopausal symptoms.

 Managing The  Signs and Symptoms of Andropause

 

What is andropause?

            Andropause is a result of hormonal changes that occur in middle-aged men.  This is primarily due to a decrease in hormone production and may also be referred to as “male menopause”, as it is the male version of menopause that occurs in women.    

 

What causes andropause?

             Andropause generally occurs between the ages of 35 and 60 and is a result of diminishing levels of testosterone and DHEA.  DHEA is a precursor to testosterone and other hormones and has been described as “the fountain of youth” hormone.  Men lose approximately 1-3% of their testosterone each year. 

 

What symptoms are associated with andropause?

What treatment options are available?

          Andropause can be treated with lifestyle modifications, which include diet and exercise, and/or natural hormone replacement therapy.  Testosterone can be administered several ways including topical, intramuscularly, and oral troches. 

Testosterone is not well absorbed when swallowed orally due to what is known as a “first-pass” effect, in which the liver metabolizes more than 80% into an unusable form before it’s even absorbed into the bloodstream.  Topical gels are absorbed through the skin, while troches are absorbed through the cheek.  Gels and troches are preferred over injections by most patients and both have very high rates of absorption. 

 

How do I know if my prescription is working?

        Our products are custom compounded specifically to your needs and can be titrated (increased or decreased) by your physician based on your individual response to therapy.  It usually takes two to four weeks after initiating therapy to reach therapeutic levels in the bloodstream.  A dose increase is usually attempted first if there is no response after four weeks.  In addition, a saliva test can be performed by either your physician or we can send a saliva sample off to a laboratory for you to determine your testosterone levels.  

 

What side effects may I experience with use? 

        Possible side effects with use of testosterone include, but are not limited to acne, excessive hair growth, deepening voice, and mood swings/rages.  Should this occur, a decrease in dose usually resolves side effects.  For those patients who have sensitive skin or are allergic to egg whites and may experience skin reactions, an alternate base can be substituted.  Irritation of the skin may occur with repeated application to the same area, so it is important to rotate the area of administration.  Irritation of the gums or cheeks may occur with use of the troches.   

 

 

Ketoprofen Transdermal Topical Gel

An Anti-Inflammatory for the relief of symptoms of arthritis and or muscle or joint pain.

Your Webb’s pharmacist can compound a gel that may provide relief from the symptoms associated with chronic arthritis.  It is called Ketoprofen Gel and has been used extensively by compounding pharmacists with great success and may be just the appropriate alternative for you.    

Ketoprofen Transdermal Topical Gel is topically applied, custom made product that can help alleviate pain and inflammation associated with arthritis, tendonitis, muscle sprains, or other types of injuries in which inflammation is a major component.   

Advantages Ketoprofen gel offer over oral products include:

ü      Enhanced penetration into the affected joint(s) or inflamed area(s)

ü      Ability to add other active ingredients such as muscle relaxants if needed 

Directions For Use:  Thin film (usually dime sized amount, depending on size of inflamed area) rubbed thoroughly into affected area(s) every eight hours.  Make sure the skin is clean prior to administration.  Relief may occur in as little as twenty minutes.

Precautions:   You should not use this product if you are allergic to Ketoprofen or other anti-inflammatories.  This product contains lecithin and patients who are allergic or sensitive to egg whites may have skin reactions to this product.

Dispensed:  In a 10, 20, or 30% PLO gel in 30 or 60 ml syringes for easy measuring and application.

Scopolamine Gel  

For Motion Sickness

 

Transderm-Scop® patches are used for motion sickness. One transdermal patch placed behind the ear provides up to 72 hours relief from travel sickness. These manufactured patches might not be suitable for all patients. Some patients may need more or less of a dose or require greater dosing flexibility than the long acting patch allows.  With a Doctor’s order, a suitable replacement is now available for patients experiencing nausea or vomiting from diverse causes such as motion sickness.

Scopolamine 0.25mg/0.1ml in a base of Pluronic/Lecithin Organogel gives the results desired by the patch but gives dosing liberties not available with the patch.

Packaging: We package the gel in 1 ml tuberculin syringes, making it convenient for a patient to measure a dose of 0.1 to 0.2ml.  These syringes must be placed in a light resistant package, as scopolamine should be protected from light. Occlusive dressings are supplied with the gel.

Dosage:  The usual dose is 0.1ml-0.2ml applied every eight hours topically, rubbed into the skin and then covered with an occlusive dressing such as Tegaderm® or Op-site®. This dosing method allows the patient to adjust the dose to achieve the desired effect.  If the desired amount of nausea control is not achieved, more of the gel can be applied with the next dose.  If dry mouth, drowsiness, or blurred vision becomes a problem with the current dose, less gel can be applied.  The freedom to change the dose is a valuable quality of this dosage form.

 
Cold Sore Lip Balm 

 

If you suffer from frequent cold sores, we may have just the remedy you are looking for.  Cold Sore Lip Balm is a compounded product that helps to shorten the duration of cold sores caused by the herpes simplex virus (HSV-1).  One major advantage to this product versus commercially available products is a significant reduction in cost.  It is a prescription only product and thus requires a valid prescription from your physician.

 

What’s In It?  Our product contains three active ingredients that work together to quickly eliminate viral lesions and the associated discomfort.  Acyclovir is an antiviral to prevent viral replication and control spread of the lesions.  Sodium lauryl sulfate has been shown to enhance the effectiveness of the drug by increasing the absorption of the acyclovir.  Lidocaine helps relive associated pain and discomfort.  Lidocaine is optional at your physician’s discretion. 

 

How Do I Use It?  Our Cold Sore Lip Balm comes in a convenient lip balm stick that is normally applied five times per day or at your physician’s discretion.

 

Are There Any Side Effects?  Burning and/or dry lips are the most common side effects.  An emollient such as CarmexÔ is recommended to use in conjunction to keep the lips from drying out and cracking, which can result in greater susceptibility to infection.

 

Precautions:  Do not share this product with others, as the virus is contagious and can be easily transmitted to others.  Use this product only as directed by your physician.

 

Compounding Tidbit

What is a PLO Gel?

Many of our topical products are compounded in a pluronic lecithin organogel base referred to as PLO gel.  PLO is an emulsion that has the appearance and feel of a gel.  It consists of both an aqueous and a lipid (fat soluble) phase, which causes the formation of micelles.  The PLO base allows a drug to penetrate the skin without damaging it, so that the active drugs can easily enter into the systemic circulation.  One unique characteristic of this base is that it is thermo reversible.  It should be stored at room temperature to keep the emulsion solid; if refrigerated it quickly turns to liquid. 

Creams, Ointments, Gels or Liquids

Occasionally your Doctor may desire to formulate a cream or ointment specific to your condition. Many times Physicians want to combine several active ingredients to meet your specific needs. Or perhaps they want an unavailable strength of a drug added to an existing product (or a special base) to meet a particular need. Whatever the situation we will make every attempt to prepare your compound to your physicians exact specifications.
